This is a Part-Time position
Shift : 20 hrs a week (.5 FTE) Mon. - Fri. must be flexible occasional Sat. & Sun.
Under the supervision of the Director Center of Excellence in Health and Wellness, incumbent is responsible for providing comprehensive Wellness assessments and developing personalized Wellness plans to the members of the Center of Excellence in Health and Wellness. Incumbent will provide ongoing support and coaching to help members with their Wellness plans. Incumbent will serve as a member of the interdisciplinary team at Broadmead providing support, information and connections to the Centers of Excellence.
Provides a comprehensive Wellness assessment for each member of the Center of Excellence in Health and Wellness.
Assists in the development and selection of the Wellness assessment tools and protocols for the Center of Excellence uses for Wellness assessments.
Regularly evaluates assessments tools and makes recommendations for changes.
Develops and implements an individualized Wellness plan for each participant member. Consults the interdisciplinary team when appropriate.
Provides ongoing coaching and support to members to help them with their personalized plan and assist them with any additional needs.
Serves as a liaison between the Center of Excellence in Health and Wellness and the Center of Excellence in Dementia Care. Collaborates where possible to ensure care plans from both Centers are implemented successfully.
Serves as a liaison between the Center of Excellence in Health and Wellness and the medical office at Broadmead to help members in areas where Wellness and medicine intersect.
Assists members, where appropriate, by coordinating making connections with resources and services in the greater community that can improve their Health and Wellness.
Provides educational support to members that can help them cope with changes or challenges with their health and wellness.
Attends and/or participates in pertinent medical team and rehab team conferences regarding resident member’s progress, problems and needs.
Acts in compliance with Broadmead regulatory and professional standards and guidelines.
Assesses continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency.
Adheres to facility policies and procedures and participates in facility quality improvement and safety programs.
Designs and implements educational presentations.
Assists in Center of Excellence special event design and implementation.
Attend scheduled departmental meetings as necessary.
Perform additional duties and projects as directed by supervisor.
Graduate of an accredited school of nursing
Valid license as registered nurse or licensed practical nurse in Maryland.
Minimum of two (2) years of experience.
Requires strong coordination and program management skills.
Knowledgeable of a variety of assessments for health and wellness.
Knowledgeable of care management strategies and overall resources for older adults.
General knowledge of human behavior practices.
Requires ability to work independently and with others.
Requires the ability to work well with older adults who may sometimes exhibit confusion, impaired hearing or sight, or unusual behavior.
Communicates effectively (verbal and written) with members and staff.
Current Driver’s License
Has latitude to cope with most problems
Confers with Director Center of Excellence in Health and Wellness on unusual issues
Ensures necessary and required documentation through timely and accurate record keeping.
Maximize effectiveness of activities through careful planning, implementation, and follow-up on an ongoing basis
Maintains any applicable certifications.
