The Patient Care Coordinator is responsible for the oversight and participation of patient’s care of ongoing BioPlus pharmacy services.

Responsibilities

• Coordinate monthly deliveries of specialty medications; ensuring right patient, receives right medication to the right address

• Achieve monthly call calibration meeting metric of 90

• Communicate via phone, and/or email with patients, physicians, and payers to provide them necessary information regarding patient delivery or delay of medication delivery

• Responsible for documenting all communication regarding patient treatment and/or physician office in patient medical record

• Assess patients therapy compliance and report noncompliance of treatment to pharmacist and physician

• Document ADR/Adherence occurrences and escalating all clinical calls to appropriate clinicians

• Adjudicate patients claim and bill patient for co-payment during the refill process

• Intake and/or enrollment of patients

• Participate and perform other duties as assigned by leadership

Skills & Abilities

• Knowledge of standard medical terminology

• Ability to handle challenging patients/vendors in a professional manner

• Ability to learn quickly, solve problems, and make decisions to assist our patients

• Demonstrate excellent oral, presentation, and written communication skills

• Demonstrate proficiency in the use of MS Office software, which includes Excel, Word and Outlook

• Demonstrate the BioPlus C.A.R.T. values

Requirements:

• High School diploma or equivalency

• Comfortable in a phone based, office environment

• 2 years of specialty pharmacy experience preferred
