Bay State Community Services is a not-for-profit human services agency, dedicated to improving the social functioning of adults, adolescents, and children within a framework that recognizes the primary importance of family and community. To this end, the agency's mission is to support individual, family, and community empowerment through an integrated, culturally competent continuum of social, mental health, addiction, and correctional services.
To integrate and balance wraparound practices and managed-care methodology. Responsible for collaboration with a variety of providers and community supports. Responsible for intake and preparation of all new families to the network. In addition, you will be responsible for facilitation of treatment planning teams, ensuring that service implementation and coordination occurs as well as the utilization management process.
The Care Coordinator is Responsible for:
The care coordination of services to youths and their families, in adherence to the wraparound philosophy of care, working with multiple systems of care, monitoring the delivery of services to families, case management, assessment to families and facilitating team meetings.
Ability to travel within designated catchment area.
Master's (MA, MS, MSW, M.Ed.) Degree in psychology, counseling, social work, marriage and family therapy and/or a human services related field is preferred; Experienced Bachelor's (BA, BS, BSW) Degree level candidates with experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems.
The candidate should demonstrate strong organizational skills, an ability to multi-task and able to meet deadlines.
Bilingual in Spanish is desired.
A MA driver's license, a clean driving record and a personal vehicle is required. An acceptable CORI and SORI background record check as required by program.
