BAYADA Hospice has a new opening for a Full-time Registered Nurse Case Manager with a heart for hospice and palliative care. 

Focusing on one-on-one care at the end of life, this Hospice Nurse Case Manager position is for visits in Philadelphia county.  As a Hospice RN, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing, psychosocial care, music therapy and veterans services to clients, affording them the opportunity to receive comfort and dignity.

The Role of an RN Case Manager:

• Assessments
• Training and education of family members
• Medication administration
• Communication with other members of the client's multidisciplinary team
• Supply management
• Other assigned duties related to case management

• Have a current license as a Registered Nurse in the state of Pennsylvania
• Have at least one year of verifiable work experience as an RN, preferably in hospice or palliative care
• Graduate of an accredited and approved nursing program as indicated by school transcript or diploma, and
• Demonstrate an ability to read, write, and effectively communicate in English, and
• Achieve a passing score on written examinations, and
• Have a criminal background check completed, and
• Meets all competency, experience, and educational requirements in accordance with BAYADA Qualifications and Code policies as applicable to care and services to be performed.
