Independent and assisted living  senior housing with services.

This part time position (24 hours including weekends) is responsible for managing the residents’ health plan and monitoring the day-to-day nursing activities during the shift in such a way that meets the needs of the residents, complies with the Home Health Program policy and procedures and with the mission, goals and policies of Avinity.

• Current Minnesota Licensure as a Licensed Practical Nurse.
• 2 years experience in home care or assisted living required.
• 1 year experience in monitoring HHAs preferred.
• Must meet and maintain the requirements of the Department of Human Services background check for health providers. 
• Must meet and maintain the requirements of Avinity’s criminal and drug testing background check policies.
• Must meet and maintain the physical requirements of the position.
• Understand and communicate with specialized medical terminology and the ability to explain procedures to residents, family members.

• Ability to use appropriate technology including computer, internet and software.
