Full-time: Monday- Friday 8am- 5pm, rotating on-call rotation
LIFE (Living Independence for the Elderly) is a Medicare/Medicaid program that helps people meet their healthcare needs in the community instead of going to a nursing home or other care facility. Job Description
- Ensures ongoing coverage and continuity; coordinates with other physicians for on call coverage and participates in that coverage as needed.
- Acts as medical liaison between LIFE program and others in the medical community (i.e. primary physician groups (attending), specialists and hospital staff); promotes discussions regarding medications, treatments, discharge and alternative regimens.
- Ensures compliance with LIFE provider regulations as they pertain to medical/clinical components.
- Utilize your advanced clinical skills to accurately diagnose and treat a wide range of medical conditions, helping patients on their journey to recovery and wellness.
- Compassion, patience, and genuine desire to make a positive impact on the lives of seniors.
Qualifications
- Nurse Practitioner with MSN and licensed to practice in the Sate of PA.
- Valid Pennsylvania Driver’s License.
- Minimum of one year of experience in care of a frail or elderly population.
- Experience working with an interdisciplinary team and performing comprehensive geriatric assessments preferred.
- Demonstrates leadership abilities and the ability to coordinate and facilitate teamwork and interdisciplinary groups. Demonstrates positive interpersonal relations in dealing with others, so that productivity and positive interpersonal relations are maximized.
- Consistently demonstrates and incorporates principles of safety and infection control into daily activities.
