Seeking CNAs who can work in our long term care facilities. 

DUTIES:

*Provide care and tend to physical needs of the patients according to the patient care plan;

*Perform and document rounds of patient receiving care and report changes in patient condition to the Nurse;

*Obtain specimens for diagnostic testing;

*Complete all required documentation of patient care, provide written and/or oral reports;

*Clean, maintain, and check equipment;

1. Resume

2. State ID or Driver License

3. SS card

4. NY State CNA license and registration

5. Two reference letters

6. Physical exam with PPD

7. MMR, varicella titers lab results and Hep B lab result

8. Drug screening lab works in the last 10 months

9. CNA school diploma or completion

10. CPR (optional for CNA)

11. Malpractice Insurance (optional for CNA)
