Director of Case Management

Job Description

We are working closely with a top-rated health system in Northern Los Angeles searching for an experienced Director of Case Management to oversee inpatient care coordination, hospital transfers, and discharge planning.

The group is growing fast, and the ideal candidate for this role will be a Registered Nursewith 5+ years of outpatient. complex, or ambulatory Case Management or Utilization Management leadership experience.

Essential Functions:

  • Responsible for the hiring, training and supervision of managers and case management staff
  • Responsible for annual budget development, new program development, establishing department policies and procedures, department goals and objectives
  • Promotes and coordinates department Quality / Performance Improvement and coordinates activities with other departments to provide and maintain optimal standards of patient care
  • Assurance that the staff adheres to the rules and regulations of all local, state and federal guidelines and to the standards of accrediting bodies
  • Works with Administrative Teams, Medical Directors, and Utilization Management to set short-term and long-term goals
  • Creates and implements systems of communication between other departments and contracted hospitals that increases the efficiency and effectiveness of the repatriation case management processes

Main Requirements:

  • Minimum of 3 years’ experience in a Case Management leadership position (Manager or Director level)
  • 2+ years of related experience (Case Manager, Care Manager, Case Management, Care Management, Utilization Review, Utilization Management, Care Coordination)
  • Current California RN License

Compensation is at the discretion of the employer and dependent upon experience and education.
